Report your Changes to your County

If you move to a new address or have a change in your income, you need to report it to your county right away. Call your county Office of Human Services to report the new address. You can also report the change through PEAK (coloradopeak.force.com).

Changes you will need to report:

  • New address
  • New job or lost job
  • Pregnancy or new baby
  • Shelter costs (rent & utilities)
  • Household changes

Renew your Benefits On Time

Every 6 months, you will need to apply again for SNAP. Your county office will mail you the form to fill out and return to them, or you can re-apply online through PEAK (coloradopeak.force.com).

Important: do not forget to sign the renewal. You may be required to complete an interview with your county office. If they require this, they will be required to provide interpretation for you if needed.

Renewal Options:

  • Return the form by mail
  • Use PEAK to complete renewal
  • Return form to county office in person
  • Call the county office for help if needed

Submit Proof (verifications)

Sometimes, SNAP requires you to verify your income by submitting proof to your county office of the income you reported. If this is required, you will receive a letter in the mail telling you which items to turn in, and when they are due. Turn them in on time.

Proof Submission Options:

  • Mail a copy of the proof to the county office
  • Upload the proof through PEAK
  • Bring the proof to the county office
  • Call the county office for help if needed
