Services Provided by the Health Team

Main Health Services

Health Intake and Health Screening Referrals
We complete intake information with every family upon arrival or beginning of services to assess your health needs. Once intake is complete, a referral to the refugee clinic is submitted to connect families to the domestic medical exam and primary care services.

Coordinate Pre-Screening or Triage Appointments 
For clients with more urgent health needs upon arrival, the health team coordinates pre-screening or triage appointments to get the urgent need addressed. For those without transportation support, IRC can help arrange transportation for the pre-screening/triage appointment only.

Coordinate Health Screenings or Domestic Medical Exams
We work with clinics to coordinate health screenings/domestic medical exams once referrals have been submitted. We help make sure families are notified when screenings have been scheduled and assist in transportation for screenings.

Health Navigation
After health screening, the health team can assist clients in navigating any initial specialist referrals or tuberculosis referrals. All future appointments will be the responsibility of the client/patient, not IRC.

 

Additional Services

Health Education
We offer individualized and group health education classes to help clients learn how to navigate the health system on their own. Topics include: primary, urgent, and emergency care; how to make appointments and ask for interpretation; Medicaid benefits; how to schedule Medicaid transportation; how to resolve medical bills; emotional health resources; and other primary care services.  

Health Clinic Bus Orientation
We offer bus orientations to teach clients how to take the bus to their primary care clinic. Clients not eligible for Medicaid transportation services must take the bus or find their own way to get to the clinic.  

Medical Case Management (only for those with complex medical issues
We provide intensive medical case management for clients with complex medical issues. This includes assistance/education on booking Medicaid transportation, coordinating with specialty clinics and hospitals on re-occurring appointments or surgeries.
